How to Claim
- Visit any contracted healthcare provider with your membership card
- Present your card at registration for verification
- Receive medical services covered under your plan
- The provider submits claims directly to Phoenix Health
- Claims are processed and paid within 30 days
Refund Policy
A member can apply for a refund where they could not access a medical service they are entitled to at a provider site and had to make an out-of-pocket payment elsewhere.
All full refunds will be made for covered services within 10 working days from the date of submission.
