How to Claim

  1. Visit any contracted healthcare provider with your membership card
  2. Present your card at registration for verification
  3. Receive medical services covered under your plan
  4. The provider submits claims directly to Phoenix Health
  5. Claims are processed and paid within 30 days

Refund Policy

A member can apply for a refund where they could not access a medical service they are entitled to at a provider site and had to make an out-of-pocket payment elsewhere.

All full refunds will be made for covered services within 10 working days from the date of submission.
